Referral process

It is hoped the information below will provide clarification to those professionals who recognise when a child and their family could greatly benefit from accessing Tŷ Hafan’s services.

Our referral criteria is as follows:

  • A child or young person must be life-limited. This means there is a strong possibility of the child or young person dying before the age of 19. The child’s life is shortened by either a life-limiting condition or a cluster of symptoms for which there is no reasonable hope of cureWe also ask you as a professional "would you be surprised if this child or young person died before they reached 19 years of age?"

  • If you think the child or young person is life-limited and you wouldn’t be surprised if they died before the age of 19, then Tŷ Hafan can offer palliative care services to meet the physical, social, psychological and spiritual needs of the child and family.

Routine referral process

Give us a call, send an email or write us a letter, or perhaps let us know when we see you next.


It is important that referrals have the consent of the child or young person (where possible) and the person with parental responsibility.


Tŷ Hafan’s Palliative Care Nurses are always available to discuss potential referrals and to help you support the families involved at what is a demanding
and distressing time for them.
