If the parents of a child are happy for them to be referred to Tŷ Hafan, they need to complete a form giving us permission to ask their paediatrician for a medical report.
We may then need to carry out an assessment of a child or young person before offering support, which means our services are not always immediately accessible. However, we do have an emergency referral process which allows us to offer immediate support, if this is required, e.g. for a child needing end–of–life care.
If a child or young person is eligible to use our services, we’ll then identify their needs and those of their family members to help create a personalised care programme.
